Wegovy (semaglutide 2.4 mg weekly) slows gastric emptying and reduces appetite [2]. Food sits in your stomach longer than it used to. Eat the wrong things in the wrong quantities and you will feel awful. Eat the right things in the right quantities and the drug does what it was designed to do, which in the STEP 1 trial was an average 14.9% body weight reduction over 68 weeks versus 2.4% on placebo [1].

The eating strategy in one screen

Five rules cover most of what matters.

  1. Protein first. Aim for 0.7 to 1.0 g of protein per kg of body weight per day, eaten in front-loaded portions across two or three meals. Protein blunts the muscle loss that happens whenever weight comes off fast [3][4].
  2. Smaller portions. Most people on Wegovy eat 30 to 50% less food than they did pre-medication and feel fuller doing it. Trust the signal. Stop when you feel comfortable, not when the plate is empty.
  3. Lower fat, especially fried. Fat slows gastric emptying further. Wegovy is already doing that for you. Stack the two and you get nausea, reflux, and the dreaded "sulfur burps."
  4. Hydrate hard. Two to three liters of water a day. Thirst signals get suppressed along with hunger signals, and constipation is the most common GI complaint on semaglutide.
  5. Skip or limit alcohol. Wegovy slows alcohol clearance, amplifies its effects, and shares a nausea pathway with it. Most people who keep drinking on Wegovy regret one specific evening within the first three months.

Protein: the macro that protects your results

When body weight drops, lean mass drops with it. A STEP 1 body composition substudy found that roughly 39% of the weight lost on semaglutide came from lean tissue, with the rest from fat [4]. That is the typical ratio for weight loss in general, but it means muscle preservation is not automatic. Protein intake is the biggest dietary lever you have to bias the loss toward fat.

The number to remember is 0.7 to 1.0 g of protein per kg of body weight per day [3]. For a 90 kg person that is 63 to 90 g. For a 70 kg person that is 49 to 70 g. Many people on Wegovy eat far less than this because their total food intake collapses and protein gets crowded out by carbs and small snacks.

Protein-dense foods that work well on Wegovy because they are filling and easy on a slow-emptying stomach:

  • Eggs (whole, scrambled, hard-boiled). About 6 g protein each.
  • Greek yogurt (plain, low-fat). 15 to 20 g protein per cup.
  • Cottage cheese. 25 g protein per cup.
  • Chicken breast, grilled or baked. 30 g per 100 g serving.
  • White fish (cod, haddock, tilapia). Lean, mild, easy to digest.
  • Salmon and tuna. Fattier but the fats are omega-3s, so they are a different category from fried-food fat.
  • Tofu and tempeh.
  • Lentils and beans (start small, fiber load is real).
  • Whey or pea protein powder added to smoothies on low-appetite days.

If you can only finish a small meal, make the protein the first bite. Get those 20 to 30 g in before your stomach decides it is done.

Foods to minimize or avoid

Wegovy does not technically forbid any food. But the following categories cause the most problems for the most people, based on the side effect profile in STEP 1 and on standard GI symptom literature [1][2].

Carbs are not the enemy

Wegovy is not a low-carb drug. The STEP trials did not prescribe a specific macronutrient ratio. Participants were given a 500 kcal/day deficit and modest activity counseling, nothing else [1]. What carbs you eat matters more than how many.

Useful carbs on Wegovy:

  • Oatmeal. Soluble fiber, slow burning, easy on the stomach.
  • Sweet potatoes. Vitamin A, fiber, mild flavor.
  • Quinoa, brown rice, farro. Whole grains hold satiety longer than refined.
  • Whole grain bread in moderation. One slice with eggs is a reasonable breakfast.
  • Fruit. Berries, bananas, apples, watermelon are all fine. Pair with protein when possible.
  • Beans and lentils. Both protein and fiber. Start with half-cup portions because the fiber load can cause gas during titration.

Carbs to limit, less because of the carb itself and more because of what comes with it:

  • Sugary cereals and pastries (sugar plus refined flour, no protein).
  • White bread sandwiches piled high (volume problem).
  • Sweetened yogurts marketed as "healthy" (often 20+ g added sugar).
  • Pasta in giant portions with cream sauce (volume plus fat).

A normal portion of pizza, a small bowl of rice with stir-fry, a banana, a piece of toast: none of these are going to break your results. The total calorie balance over the week is what moves the scale, and on Wegovy the appetite suppression handles most of the math for you.

A realistic day of eating

This is not a meal plan PDF with macros calculated to the gram. It is what an actual day looks like for someone on Wegovy who is past the worst of the side effects and is targeting protein-forward eating.

Breakfast. Two scrambled eggs, one slice of whole grain toast, half an avocado. About 20 g protein, eats in five minutes, sits well.

Alternative: Greek yogurt with berries and a tablespoon of almond butter. 20+ g protein, no cooking.

Lunch. Grilled chicken salad with mixed greens, cucumber, tomato, feta, olive oil and lemon. About 30 g protein. Skip the croutons if volume is an issue.

Alternative: Tuna or salmon, a few crackers, sliced apple. Small, protein-dense, portable.

Snack (if needed). A handful of nuts, a hard-boiled egg, a piece of fruit, or a cup of cottage cheese. Many Wegovy users skip snacks entirely because they are not hungry.

Dinner. Baked salmon, roasted sweet potato, steamed broccoli. About 35 g protein. The whole plate is small, maybe 6 to 8 ounces of food total.

Alternative on a low-appetite day: a protein smoothie with whey, banana, spinach, and milk. Half a cup of cottage cheese on the side if you can manage it.

Fluids. Two to three liters of water through the day. A cup of coffee in the morning is fine. Herbal tea in the evening helps with bloating for some people.

Total protein: 70 to 90 g. Total calories: roughly 1,200 to 1,500, which is where most people on Wegovy naturally land without trying.

Eating through side effects

The most common Wegovy side effects are nausea, vomiting, diarrhea, constipation, and reflux [2]. They are worst during titration (the first 16 to 20 weeks when the dose steps up) and ease at maintenance.

Nausea. Eat smaller meals, more often. Avoid lying down for an hour after eating. The BRAT framing (bananas, rice, applesauce, toast) is the classic recovery menu when you cannot face anything else. Ginger tea, peppermint tea, and plain crackers help many people. Ice pops or popsicles count as fluid intake on the worst days.

Vomiting. Stop eating solids for a few hours. Sip clear fluids. Resume with bland foods (broth, toast, banana). If vomiting persists more than 24 hours, call your prescriber, both for the medication question and to rule out something Wegovy is masking, like gallbladder pain.

Constipation. This is the most underdiagnosed side effect. Wegovy slows the whole GI tract. Counter it with fluid (the big one), fiber (soluble fiber from oats, chia, psyllium husk is better tolerated than insoluble fiber from raw vegetables), and movement. A daily walk does more than most supplements. If three days have passed with no bowel movement, a stool softener is reasonable. Talk to your prescriber before adding stimulant laxatives.

Reflux. Smaller meals, finish eating three hours before bed, sleep with the head of the bed slightly elevated. Cut the fat content of your meals. Coffee and alcohol both worsen reflux for many Wegovy users.

Sulfur burps. These come from food sitting in a slowed-emptying stomach and fermenting. The fix is the same as for nausea: smaller, lower-fat, less sugar.

Hydration: the most underrated lever

Wegovy suppresses thirst the same way it suppresses hunger. A lot of people on the medication walk around chronically dehydrated and assume the resulting headaches, fatigue, and constipation are drug side effects. They are partly, but hydration is the variable you can fix in a day.

Target: roughly 2.5 to 3 liters of fluid per day for an average adult. More if you are active, in heat, or losing fluid from GI side effects. Plain water is fine. Adding electrolytes (sodium, potassium, magnesium) on days when you barely ate helps. Watery foods like cucumber, watermelon, soup, and yogurt count toward total intake.

How to know if you are getting enough: pale yellow urine, three or four bathroom trips a day, no dizziness when you stand up. Dark urine and constipation together are the signal to drink more, today, before anything else.

The alcohol question

The Wegovy label does not prohibit alcohol [2]. Real-world experience says go slow.

Three reasons alcohol is rough on Wegovy:

  1. Slowed gastric emptying means alcohol enters your bloodstream more slowly but stays longer. Tolerance drops.
  2. Alcohol and Wegovy share the same nausea pathway. A single drink can trigger a much worse hangover than you remember.
  3. Alcohol slows fat oxidation and disrupts sleep, both of which slow weight loss.

If you are going to drink: one drink, with food, not on an empty stomach, not the day after a dose escalation. Skip mixers loaded with sugar. Wine in small amounts is usually the easiest to tolerate. Hard liquor neat hits hardest. Beer is the worst combination for many users because the volume plus carbonation triggers reflux on top of the alcohol.

Supplements worth considering

Not required, but worth talking to your prescriber about:

  • Protein powder (whey or pea). On low-appetite days, a shake covers 25 to 30 g of protein in something you can actually finish.
  • Creatine monohydrate, 3 to 5 g daily. The best-studied supplement for preserving muscle and strength during weight loss. Not specific to Wegovy, just smart during any rapid weight loss phase.
  • Vitamin B12. Long-term GLP-1 use can lower B12 in some users. A standard B-complex or B12 alone is cheap insurance.
  • Vitamin D and magnesium. Most people are mildly low on both anyway. Magnesium also helps with constipation.
  • Iron. If you eat less red meat, iron intake drops. Watch ferritin levels at your annual labs, especially for menstruating women.
  • Electrolytes. On heavy GI side effect days, sodium and potassium replacement matters.

Do not stack pre-workout drinks, weight loss supplements, or appetite suppressants on top of Wegovy. The medication is doing the appetite work. Adding stimulants increases heart rate, blood pressure, and the risk of overshooting into dangerous calorie restriction.

What changes when appetite returns

Many people hit a sweet spot in the middle of titration where appetite is suppressed exactly enough to make weight loss easy. Then somewhere in maintenance, appetite returns partially. This is normal. The drug is not failing.

When appetite comes back, the work shifts from "remembering to eat enough" to "managing portion sizes intentionally." The same five rules at the top of this page still apply. Protein first. Smaller portions. Lower fat. Hydration. Watch the alcohol. People who maintain weight loss on Wegovy long-term are the ones who treated the titration phase as practice for new eating habits, not as a temporary detour they would return from.

Can you lose weight on Wegovy without dieting?

Yes, partially. STEP 1 enrolled people on a modest 500 kcal/day deficit prescription plus 150 minutes per week of activity, but adherence to those targets was imperfect and the trial still produced the 14.9% weight loss result [1]. The drug does most of the work. Diet quality determines two things: how you feel during the process, and what the composition of your weight loss looks like (more fat versus more muscle).

You can lose weight on Wegovy eating poorly. You will feel worse, lose more muscle, and rebound harder when you stop. Eating the way this page describes is the difference between a frustrating year and a year you want to repeat.

A short shopping list that makes the first month easier:

  • Eggs, Greek yogurt, cottage cheese, chicken breast, canned tuna, frozen white fish, frozen salmon fillets.

Wegovy is a powerful tool. What you put on the plate while it is working determines whether the result is a healthier version of you or just a lighter version of the same problems. Protein first, smaller portions, less fried food, more water. That is the whole job.
